We believe that people with diabetes can live healthy, productive and enjoyable lives. The Diabetes Center offers a variety of educational programs to help people with diabetes feel better and enjoy a healthier life by learning how to control their blood glucose. Our registered nurses and dietitians work with individuals and their physicians to develop personalized diabetes care plans that teach them how to:
- Plan meals
- Manage their medications
- Begin an exercise program
- Monitor blood glucose
- Make lifestyle changes
- Cope with diabetes
Why do you need self-management training?
Diabetes Mellitus is a chronic illness. Continuing medical care and education is needed to prevent acute complications and reduce the risk of long-term medical problems. A critical element in the successful treatment of people with diabetes is participation in a comprehensive
self-management care and education program, like the one we offer at the Diabetes Center.
Our Classes/Locations
The Diabetes Center's outpatient education classes are focused on controlling diabetes through self-management training. Most patients are scheduled for a group program, a total of 10 hours, over three consecutive weeks. Individual appointments are also available.
Our curriculum covers:
- Diabetes disease process
- Nutritional management
- Physical activity
- Medications
- Monitoring
- Prevention, detection and treatment of acute complications
- Prevention, detection and treatment of chronic complications through risk reduction
- Goal setting and problem solving
- Psychosocial adjustment
- Preconception care, management during pregnancy and gestational management
We also offer specialized instruction for:
Pregnancy/Gestational diabetes
Insulin pump management
Support Groups
People with diabetes and their families can exchange information, feelings and ideas at the Diabetes Center's monthly support groups, which are led by a registered nurse. Meetings are held at Florida Hospital Ormond Memorial and Florida Hospital Flagler . No registration or fee is required. Meetings do not take place in July or August.
ADA Recognition
Our diabetes education program is recognized for excellence by the American Diabetes Association. The ADA Education Recognition is a voluntary process that assures approved education programs have met stringent national standards. According to the ADA, programs that achieve recognition status have a staff of knowledgeable health professionals who can provide state-of-the-art information about diabetes management for participants. Pre-Diabetes - What is it and what can I do?
Before people develop type 2 diabetes, they almost always have "pre-diabetes" - blood glucose levels that are higher than normal but not yet high enough to be diagnosed as diabetes. At least 20.1 million people in the United States (21.1% of the population), ages 40 to 74, have pre-diabetes. Recent research has shown that some long-term damage to the body, especially the heart and circulatory system, may already be occurring during pre-diabetes.
But research has also shown that if you take action to control your blood glucose when you have pre-diabetes, you can delay or prevent type 2 diabetes from ever developing.
Diabetes facts you should know
Diabetes is a serious disease that can lead to blindness, heart disease, strokes, kidney failure, and loss of a limb. You are at greater risk for diabetes if you are age 45 or older, you are overweight, you have high blood pressure, or if you have a family history of diabetes.
